Lung US Journal Club Part 1 with @nobleultrasound and @aliteplo.  #foamed.  Lancet article discussed with first author, Christian Laursen.  | File Type: video/x-m4v | Duration: 31:16

Point-of-care ultrasonography in patients admitted with respiratory symptoms: a single-blind, randomized controlled trial. The title is a mouth full and when you read it it’s definitely a brain full.  This is an incredible study with some incredible results.  If you just read the abstract you see that the correct diagnosis is made 88% of the time within 4 hours with ultrasound used and only 63% without ultrasound.  BIG WIN! However, when you read the paper, you notice some very head-scratching things.  There may be trends towards more diagnostic testing, more treatment, and possibly increased mortality and morbidity when ultrasound is used.  So a lot of people have talked about this and wondered if it was real. Well, we wanted to dive even deeper and talk with the author of the paper.  Let’s dissect the paper and discuss what wasn’t in it.  What are we missing? Luckily, Vicki Noble and Andrew Liteplo have helped us out.  Here is the fruit of our labor……actually the first part of the fruit of our labor.  We’ll harvest the rest of the fruit for you in just a few days.  Patience….. One other reminder.  If you haven’t commented on or rated the podcast, please do, but only if it’s positive and you can give us 5 stars.  Email us if you can’t give us a 5 and we’ll try to do better.  It takes you

 Intussusception part 2 with @pemedpocast.  Also, more info on www.yellowstoneultrasound.com.  #foamed  | File Type: video/x-m4v | Duration: 16:13

Have you been scanning some little bellies for turducken?  We sure hope so.  If you’ve waiting to hear from the man, Andy Sloas, before diving in, then the wait’s over.  Here he is for Intussusception Part 2.  He does this all the time and is a scanning maniac.  Like Russ, he knows his stuff. Last week we mentioned in the show notes that we’d be doing some teaching the week before SMACC in one of the most beautiful places on earth, Jackson Hole, WY.  We’ll tell you a little more about it in this podcast.  People are already registered, though, so you better get on it.  Register NOW.  While you’re at it, go ahead and register for SMACC also……actually, I just noticed registration for that isn’t open until November.  Well, put it on your to do list.  They’re both going to be amazing educational experiences. Lastly, we told you about our Course in a Box concept last week on the podcast, but we didn’t mention where that was.  You can find a link on the homepage of the podcast now, so go check it out. Next podcast:  Lung ultrasound.  Does it lead to over-testing or is it an absolute must for anyone with respiratory distress.  We’ll hash that out with some super smart people.  Intuss with Russ!  Let’s talk about ultrasound for intussusception.  Also, want to learn ultrasound in Yellowstone?  #foamed | File Type: video/x-m4v | Duration: 30:28

You’ve got a little bugger with belly pain and they may have intussusception…..or they could just have gastro.  How to know?  Well, this isn’t the xray podcast.  Of course, you’re going to ultrasound them.  Or if you’re not, watch this and hopefully you will afterwards. Turns out ultrasound for intussusception isn’t that difficult.  There’s pretty good research on point of care providers doing it, and we’ve got Russ Horowitz from the University of Chicago on to show you how. Also, we’re teaming up with Rich Levitan in Jackson Hole, WY at Jackson Lake Lodge next year to put on an ultrasound course at the same time as his airway course.  It’s an incredible place, and it’ll be an incredible course!  Register and find out at www.yellowstoneultrasound.com.  It’s the week before SMACC next year, so a perfect bonus course if you happen to be over here anyway.  Of course, if you’re a member of the Ultrasound Leadership Academy, this course is included in the Academy.  See you there!   TEE part 3, new resus project with @emcrit and @criticalcarenow, and new joke of the week! #foamed  | File Type: video/x-m4v | Duration: Unknown

WOW, lots going on here.  Would give you a long description of each……but why don’t you just listen.  I’ll sum it up. 1.  TEE Finale:  This is it.  Done with TEE.  Mike describes how exactly to get started using this with a lecture straight from The Ultrasound Leadership Academy. 2.  Resuscitation Leadership Academy:  This is a project we’ve been working on with Scott Weingart and Haney Mallemat that we’re SUPER excited about. 3.  Joke of the week:  Not many people know Jody, but those who do know him understand how valuable a part of the podcast he is.  He edits, fixes things, does IT stuff…..and make us laugh (most important).  So we’re gonna start sharing him with you via a random, ridiculous joke at the end of each podcast.  Waste of your time?  Well, then just cut off before then and go do something “more important” than laugh.  TEE part 2 with @arntfield. #foamed. Also, EDECMO lab by @drbellezzo, @zackshiner, and @emcrit at www.castlefest2015.com | File Type: video/x-m4v | Duration: Unknown

OK, so here it is. Rob’s gonna show us exactly how to do this.  How to take those 800 normal Transesophageal Echocardiography (TEE) views and distill it down into a few key views.  We know you aren’t doing this yet, but we think it’s coming.  Just a matter of time.  Get ahead of the curve. And yes, you read the title right.  The EDECMO folks, Shiner, Bellezzo, and Weingart will be teaching an ECMO lab at Castlefest.  Sure, it’s not really an “ultrasound procedure”, but we’re obsessed with this cutting edge, futuristic technique of saving lives that couldn’t be saved otherwise.  So we convinced them to come out to Castlefest, mainly so we could learn it from them, but if you’re coming, you may as well learn as well.  We’re super pumped about this!  You can sign up on the Castlefest website now.  Oh yeah, Rob will be there again teaching TEE.  We’ll run some ultimate simulations with ECMO, TEE, and all kinds of things that make us as excited as a tick on a fat dog.  Bedside TEE with @arntfield Part 1.  Yes, you heard right.  Point of care providers doing TEE.  It just got real.  #foamed | File Type: video/x-m4v | Duration: Unknown

At Castlefest 2014, Rob Arntfield blew our minds with his point of care Transesophageal Echocardiography (TEE) talk.  He showed us why and how we could and should be doing point of care TEE.  Then we all practiced on simulators.  It was a phenomenal experience, and he’s agreed to come back in 2015 and do it all again.  So if you haven’t signed up, do that NOW! However, we think this is too important to just teach to Castlefestians, so we want to #foamed it out to all of you.  The truth is, this isn’t the hardest thing in the world to do.  It sounds scarier than it really is.  If you can get your hands on a simulator to practice, you can totally learn this……probably more quickly than TTE.  That’s the experience of most people who do this.  The windows are really much easier. We’ll be getting part 2 out to you very soon.  In the meantime if you want to read more, then pick up Introduction to Bedside Ultrasound that has a chapter on this written by the man himself, Robert T. Arntfield.  Or hangout with him as part of the Ultrasound Leadership Academy or come practice with him and us at Castlefest.  Cardiac valve assessment part 2 with @arntfield. GE Vscan dual probe machine update. #foamed | File Type: video/x-m4v | Duration: 22:31

What?!?!  Why is there a company mentioned in this episode!?!?  Have Mike and Matt sold out?  No, we take nothing from GE or any other company, but we’re excited about this new machine, and while we hesitated at first, we realized that this is important to our listeners and we decided to talk about it. Now, for the education.  This is the second part of the cardiac valve assessment talk done by Rob Arntfield at Castlefest last year.  He’s coming back this year and will be teaching valves again at Castlefest, so sign up now.  No reason to wait for some awesome education, though.  Learn now, start practicing, and then when you’re scanning with Arntfield at Castlefest then you’ll already know what you’re doing and can focus on the advanced stuff.  For now, let’s keep talking about the basics. Lastly, the ebooks are free on itunes this weekend (Friday and Saturday 6/20-21).  We got another request from medical students for this weekend, and we find it impossible to say no.  You may as well take advantage. Ridiculous lineup!  Go register now at www.castlefest2015.com. Now, for the meat of the podcast.  You may have NEVER even thought about looking at cardiac valves.  Maybe you thought this was beyond your scope.  Well, it’s time to expand your scope and skills.  This is a basic lecture.  We’re not going to get crazy on you.  Just the basics.  Just the can’t miss diagnoses.  If you listen and still think valves aren’t for you……fine.  But give it a listen.  We think Robert T Arntfield will change your mind. Also, we almost forgot……One Minute Ultrasound is NOW AVAILABLE on Android.  It’s free just like the iOS version.  One more thing.  Introduction to Bedside Ultrasound is free for a couple days.  Some medical students from UCI wanted to load up some iPads to use for teaching on a mission trip, so we made it free for them, but you all get to benefit as well.  Download quickly!  Multi-Organ US for PE. Castlefest 2015 registration open…..last year EVER! #FOAMED | File Type: video/x-m4v | Duration: 18:14

We’ve been watching, and you all order a lot of CTs for PE.  If the patient needs a CTA fine……but maybe they don’t need it as often as you think.  There may be a better way.  We’ve got some hot new evidence for you that could be huge for your patients who may have a PE.  Now it requires being a doctor, doing some thinking, and obviously using sound waves, but we know you.  This is right up your alley.  So listen in, read up, and then give it a shot. Also, it’s only been a couple months since Castlefest, but the castle is for sale (seriously), and it looks like this will be the last year for Castlefest.  So we’re opening it up early.  We’re slashing the number of registrants we’re taking this year as we really want to control it and make it an unbelievable educational experience this last time.  We’re getting it out to you early because it’s the podcast listeners who we really want there.  We’ll announce the instructors later, but if you’ve been before, then you can probably guess who will be there.  Social Media and Medical Education. #FOAMED talk from #ACEP13 Part 2 | File Type: video/x-m4v | Duration: Unknown

Give us your feedback.  Email matt@ultrasoundpodcast.com.  We’re giving this talk again at ACEP, and we want your feedback so it can be better. In this episode we address the money part a bit.  Why free?  I mean, seriously, free?  What’s in for me?  Well, we’ll talk about this.  We think FOAMED is a win, win, win, win. Also, we swear we’ll be back on the ultrasound stuff in a couple weeks.  Currently working on an Ultrasound in PE podcast.  There’s some hot new literature with some impressive numbers.  We’ll go over it and hopefully teach you some stuff very soon.  Stay tuned!  Social Media and Medical Education. #FOAMED talk from #ACEP13. Part 1 | File Type: video/x-m4v | Duration: Unknown

If you were at ACEP 2013 and you heard me (Matt) give this talk, then maybe you can skip this.  It’s not really an ultrasound talk after all.  This is about #FOAMED.  Social Media in Medical Education.  Mike and I both are big believers in FOAMED, Free Open Access Medical Education, and I was asked to talk about social media at ACEP.  So we wanted to put this out there for everyone on the podcast with some comments from Mike. Another reason we wanted to push this out was to get feedback.  I’ve been asked to give this talk at ACEP again in 2014, and I’d love to hear your comments.  A lot has changed in a year, so let me know what you think can be cut, what needs to be added, and so forth.  This is part 1, but we’ll be releasing part 2 in about a week.  Then we promise…..BACK TO ULTRASOUND. It’s here!  Castlefest 2014 is this week!  We hope you’re as excited as we are.  We’re going to be covering a lot of cardiac ultrasound….pretty advanced cardiac ultrasound.  So we wanted to put out a quick podcast on some of the more basic, foundational studies of bedside cardiac US. This is short and sweet.  Very minimal and not at all supposed to be an in depth look.  It’s just a few studies that we think are important background to know when we talk about using echo at the bedside.  It’s definitely part of the “Back to the Basics” series.  If you missed the FASH episode on HIV/TB last week definitely go back and watch it. Also, come back right here each day to find the link to watch all of Castlefest 2014 for free!  Lectures by Mike Stone, Vicki Noble, Chris Fox, Scott Weingart, Andy Sloas, Rob Arntfield, Haney Mallemat, Mark Goodman, and others. Tuesday and Wednesday will be Livestreamed by Rob Rogers, who’s in town for the event.  That link is HERE.  However, Monday and Thursday may be via Google Hangout or whatever works best for us.  In general the lectures will be from 0800-1000 and 1300-1500 each day, but the schedule is subject to change.  We’ll try to tweet out updates. We swore we’d bring it to you, and here it is. Yes, it’s not basic like we’ve been doing recently, but we just can’t help ourselves.  This is too cool not to bring to you now.  If you work in a setting where there is a high prevalence of HIV/TB or you ever plan on working there, then you NEED to listen to this and learn this. Also, you already know the Introduction to Bedside Ultrasound is free on Inkling.com right now.  Well it’s also free in iTunes right now.  So it’s completely FREE everywhere for a few more days.  Make sure all the medical students that you know download it.  It’s available for their ipad or mac (iTunes) or their iphone, PC, or Android via Inkling.  Don’t miss out on this #FOAMED.  Ultrasound Mistakes Part 2 by @EGLS_JFandMax, & Cabo, & HIV/TB US coming soon! #FOAMED | File Type: video/x-m4v | Duration: 15:30

You’re right, that was a lot of possible ultrasound mistakes the Canadians laid on us last time.  They’re not done, though.  There’s more.  Listen to find out.  We would tell you more about it, but we don’t want to ruin the surprise…… Also, if you haven’t heard, we’re teaching at a pretty sweet conference in Cabo in December with Weingart, Orman, Sloas, and Levitan.  Find out more about that HERE and register HERE.  Very limited space at this all-inclusive conference. Coming up: We know we’re in the back to the basics series, but we can’t help ourselves.  We’re bringing you a not so basic talk in a week or so.  It’s on the FASH exam, which is a sweet, cutting edge exam for TB/HIV.  It features Hein Lamprecht, a genius South African EM doc we met at Sonosweden who taught us this.  It’s pretty awesome. Lastly, we can’t not mention SMACC.  We just got back and it was……INCREDIBLE.  What an amazing conference with amazing attendees and speakers.  It’s in Chicago next year, so be on the lookout for when registration opens up.  Hope to see you there in 2015.
