Ep. 67: Autoimmune Q&A with the Primal Girl In this episode, Stacy and Sarah are joined by Tara of PrimalGirl.com, author of The Hidden Plague, to chat about Tara's experience with adopting an autoimmune protocol and to answer reader questions on managing autoimmune conditions.The Paleo View (TPV), Episode 67: Autoimmune Q&A with the Primal Girl 0:00 - Introduction 0:46 - News & Views Both Stacy and Sarah are feeling sick and sounding froggy Stacy, while she had high ambitions for the previous week, got the flu and had to slow down - but she is on the mend now Sarah is also not feeling the greatest, so both hosts are hoping that this week's guest steals the show! Welcome back Tara from PrimalGirl.com (been on the show twice before, here and here), who is releasing an autoimmune related book called The Hidden Plague Tara had a lot of success with losing weight going primal, but she had been dealing with a skin condition for a long time and recently received a diagnosis of Hidradenitis suppurativa When Tara discovered the autoimmune connection to HS and that no one else had information on that connection, she decided to put together a resource for those dealing with the same struggles and is excited for her book to release on December 4 (8:20) HS is an autoimmune skin condition that can be mild to debilitating It manifests as painful bumps and boils that appear where skin touches skin and it looks like cystic acne If you have high levels of inflammation in your body the issue can arise anywhere there is a hair follicle There are no proven treatments for HS - there is a daily course of antibiotics or invasive surgeries available as options (10:05) A dermatologist will be able to confirm the difference between cystic acne, HS or a staff infection, but most individuals can identify HS by where it appears since cystic acne usually appears on the chest and back Sarah's recent birthday celebration didn't leave her feeling old, but her daughter turning four and developing an older personality made her feel old Sarah wanted to give a special thank you to all who have recently reached out to thank her for her posts and resources Tara is so grateful for all that Sara contributed to her autoimmune journey and not only referenced her throughout the book, but also wrote a note to her in the back of The Hidden Plague (16:57) Questions & Answers Ashanti - feeling terrible from a number of health challenges that are linked to her PCOS, will a paleo diet help? Tara did deal with PCOS prior to adopting a primal lifestyle and she found that the diet eliminated so many of her problems by regulating her insulin and hormones Tara notes that Ashanti will be able to balance her hormones by controlling her insulin through a paleo diet Sarah encourages Ashanti to test an autoimmune protocol, but if that feels to limiting at the moment, to focus on omega-3 fatty acid intake and to get glycine in your diet from gelatin Sarah also notes to focus on nutrient density instead of restrictive dieting - center your calories around foods like citrus fruit, organ meal, fish, shellfish, bone broth, vegetables, berries, pasture raised meat Do not approach it as a diet or a challenge, but as a way to eat to be healthy - find a way to include treats that doesn't hurt your body, but helps you along to your health goal so that your approach is sustainable Tara also notes to limit foods with high yeast levels, as they could cause skin issues to flair Stacy notes that some can find a lot of success from going 100% paleo, focusing on nutrient density and quality foods, and not overdoing it with eggs, nuts, etc. - at that point it can be easier to make a decision to adopt an autoimmune lifestyle if that feels like the best next step Stacy also notes that dairy should be absolutely removed Stacy also encourages a change in perspective regarding your health - adopting a nutrient-

Tara didn't experience any ill effects from her progression into an autoimmune protocol Sarah saw huge improvements when she first went paleo; when she went autoimmune she had long stretches of time without improvements, but also nothing got worse Sarah has heard from some that symptoms can get worse for one to two weeks when first making the shift, specifically gut related, which Sarah notes is related to gut bacteria imbalances If things get pretty severe or don't improve after two weeks, it is important to see a healthcare professional so that they can run some tests and check what may be happening Sleep and stress management will help everything come together as well Stacy notes to make sure that you are getting enough carbohydrates, which can impact your sleep quality (36:40) Robert - have an autoimmune condition from a childhood exposure to asbestos, would an autoimmune protocol help? Ep. 64: Will Paleo Help Me In this episode, Stacy and Sarah address a large number of health questions all centered around how paleo can impact certain medical conditions and the symptoms that accompany them.  The Paleo View (TPV), Episode 64: Will Paleo Help Me 0:00 - Introduction 0:49 - News & Views At Stacy's house they recently rolled out a no television or video game rule and things have been awesome since they have been able to engage in new ways Stacy and Matt's boys had parent teacher conferences recently and they are blown away with where they are at in their development, especially with their behavior Sarah celebrated her two year blog anniversary and also won an award last week as Top 100 Nutrition Website by the Institute for the Psychology of Eating On a recent post on The Paleo Mom, Sarah interviewed her daughter about her process overcoming her difficulty eating meat This week's show is for those who are following a SAD or a gluten-free diet who have apprehensions about adopting a paleo lifestyle and if it will positively impact their life; and the second piece of the show is for those who have adopted a paleo lifestyle who are wondering if they need to test an autoimmune protocol 22:28 - Science with Sarah: Removing Toxins vs. Adding Nutrients Finding health through food isn't necessarily about what you take out, it is equally important to look at what you add in The guidelines for paleo are centered around healing a leaky gut, and maintaining a healthy gut with a strong lining - the other big goal is to avoid inflammatory foods There is absolutely variability in the foods that will fall on an individual's list when looking at these general guidelines The habits that Stacy and Sarah advocate to add in are snout to tail eating practices, buying local variety, including seafood and organ meats, eating produce of a variety of colors, adding pasture raised eggs, etc. - seeking out the most nutrient dense foods that you can When you are adopting a paleo diet don't focus on what you are taking out, but look at what you are adding in to optimize your health and restore your nutrient demands Stacy notes that as you eat these foods more often your body begins to crave them because your body is recognizing that it's needs are being met through that food 34:04 - Questions & Answers Michelle - how will a paleo diet impact my depression? Stacy notes that she saw her and Matt's depression disappear when they healed their guts Sarah experienced the same thing and she had battled depression and anxiety for most of her adult life prior to adopting a paleo lifestyle Some things to look at are gut-brain access through omega 3 fatty acid consumption from seafood and stress management techniques (36:45) Izzadora - would paleo impact a bipolar disorder? Yes, removing grains, increasing vegetables and decreasing sugar will all have positive impacts Getting eggs and fish will help with nutrient needs Approach paleo from a nourishing perspective, not a diet mind set (39:01) Edson - will GAPS/FODMAPS/SCD or AIP impact our son's ADHD and oppostitional defiant disorder? If you are not eating paleo at all, first start there so that you don't take too much out too soon and test a slower transition process, eliminating things over time All of those diets are very specific for specific reasons, so if you don't know what is going on from a gut level it is hard to say which diet will impact you - going paleo will be a very good start Sarah agrees to focus on gluten-free, casein-free, and food dye free eating - be sure to add in seafood and broth as you eliminate those other foods (43:40) Rachel - how should I handle my macro ratio intake to help my PCOS and lose weight? Stacy suggests eating what you feel comfortable with and what makes you feel your best Make sure that you are getting a lot of non-starchy vegetables and your body will likely steer you towards the ideal protein serving Check out Paleo For Women and check out Stefani's book on PCOS Sarah also notes the importance of stress management, quality sleep, and to take a look at The Hormone Cure by Sara Gottfried Try a strict elimination of dairy to see how it impacts your goals (48:54) Melissa - how do I combat extreme fatigue when adopting a paleo lifestyle? Stacy feels that you are lacking carbs and/or fat A sugar or carb flu is normal, but is more about exhaustion and headaches, the shortness of breath is likely linked to something else Sarah thinks that the pulse and breath concerns may have something to do with a food sensitivity Increase your carbs from starchy vegetables and fruit, and if these symptoms don't subside definitely talk to a doctor (52:12) Maya - what dietary changes can we make to help with RA and IBS? Stacy has a good friend who struggled from severe RA symptoms and paleo made a huge impact on his well-being Inflammation is causing that swelling and since paleo is a low inflammatory diet it will positively impact both the RA and IBS The benefits of paleo will touch both of these issues since paleo heals the gut and reduces inflammation With any joint condition be sure to target glycine which is in broth, and DHA and EPA from seafood A low FODMAP approach can also positively influence the symptoms (56:57) Charlene - pregnancy is impacting migraines and is making a paleo lifestyle difficult to maintain; should I eat what I can now and return to paleo after the baby is born? Stacy said that it sounds like you are having trouble with fats and may not be getting enough calories right now - try replacing those calorie loses with carbohydrates Check out the show from two weeks ago on what foods you can add in while pregnant to help you through the food aversions Coconut milk and/or oil may be a good food option to add in to help with your fat intake Sarah suggests trying fat from tallow, lard, or even fattier cuts of meat or fish, like salmon - Beyond Bacon can help you with that (1:01:36) Natalie - how can I manage my son's genetic syndrome through dietary choices? Stacy feels that there is a clear gut imbalance at play and to fully commit to a clean thirty days on paleo without slips in those thirty days Occasionally may seem seldom, but they add up and if you need to heal your son's gut you need to fully remove the dairy and grains at minimum to start the healing process Don't worry about the autoimmune approach just yet, focus on paleo Sarah notes that it is not uncommon to have different dietary needs throughout the family and to have a family discussion about how to accommodate the needs and to be sensitive to your kid's needs The Sweet Potato Drop Biscuits from Beyond Bacon would be a good option, simply use sunflower flour instead of almond (1:07:44) Emelie - how can nutrition impact my keratosis pilaris? Sarah notes that it is confirmed as an immune condition and boils down to the same thing where the immune system is not working properly Test the autoimmune protocol and be sure you are getting enough sleep and managing stress Skin conditions seem to be strongly linked to stress and blood sugar regulation (1:10:56) Andrea - how can paleo impact joint issue and type 1 diabetes? Stacy feels that adopting a paleo lifestyle and increasing your egg, nut, and nightshade intake out of the gate made your joint issues flair up Broth, sleep and really taking care of your body will help with joint issues Sarah has a post on why people with thyroid disorders do not need to avoid goitrogenic vegetables if they are sufficient in iodine The autoimmune protocol should not be challenging with type 1 diabetes because it is not low carb, you simply have to moderate your fructose intake (1:18:58) Jenna - my autoimmune symptoms are in remission from my pregnancy, if I don't follow an autoimmune protocol will it backfire after pregnancy? Stacy didn't have the glucose test in her third pregnancy, she simply declined because of the results from her first two pregnancies Sarah notes that some doctors will allow you to test with whole foods as opposed to the sugar drink from the lab Let your doctor know that you are already following a diet that is close to the gestational diabetes diet because how suagr makes you feel It is also possible to get false positives with the test if you are on a low carb diet If you are following a low carb diet, make sure when you get your carbs in that they are incredibly nutrient dense and being consumed from vegetables Stacy and Sarah both feel that ox bile supplementation is safe while pregnant Sarah is fully prepared for Halloween and Stacy is preparing for her big Halloween party and a huge book signing featuring a handful of other paleo all-stars! How can I specifically address issues with heartburn? Stacy suggests not eating dairy or sugar to reduce the heartburn issues Sarah recommends focusing on fat soluble vitamins - vitamin A inparticular is incredibly helpful to repair the gut Stacy also notes that bone broth and coconut oil will help with gut repair If Stacy could get over low stomach acid, she firmly believes that others can resolve their gut issues as well with time and diligence The Poop Pageant in Practical Paleo is a great tool to utilize when identifying if you have gut issues (look at page ) Hydrochloric acid, ox bile (specifically for gallbladder function), and digestive enzymes for pancreas support are the three supplements for supporting digestion (48:33) Sarah covered the various enzymes and what they specifically target in the digestion process To go off the protein pump inhibitors you will want to talk to a healthcare professional first and identify how to incorporate digestive supplements, get adequate sleep, limit fructose, get enough omega 3's, and make sure you are following proper meal hygiene practices regarding chewing and beverage consumption (54:33) Sandy - is Dr. Ron's Smooth Operator a safe supplement? What is the best dietary approach to healing gut issues related to SIBO? Sarah covered the ingredients in Dr. Ron's Smooth Operator and what they do in the digestive system Sarah says that it might be worth trying the supplement to see if it supports your digestive health, but you could also try a straight L-glutamine supplement The difference between SCD, GAPS and FODMAPS was covered in this episode SCD and GAPS are designed to treat SIBO by eliminating starches, but you can develop undergrowth overtime - they are not a long-term, sustainable solution SCD and GAPS have not been validated in the scientific community, low FODMAP diets have very extensive studies on how beneficial it is towards a number of gut issues Sarah's recommendation to anyone with a lot of digestive issues is a low FODMAP paleo approach with proper digestive supplementation Both Stacy and Sarah experienced great results from going through a 21DSD And don't forget that you can still be paleo and take medication, there is a time and place for conventional medical treatments (1:05:39) Pam - how much of your skin needs to be exposed to the sun to absorb vitamin d? What brand of fish oil do you recommend? This week's show is about boobs, breastfeeding and weaning Stacy is a breastfeeding advocate with a wealth of information from being a very active member of La Leche League Sarah also has a lot of experience from her years of breastfeeding her daughters, her aunt was also a lactation consultant (12:52) Science with Sarah - The Health Benefits of Kombucha Most of the health claims about kombucha have little to no evidence to support the claims However, since kombucha is a fermented beverage with probiotics there are health benefits from the beneficial yeast and bacteria strains You will also get a wide variety of unique strains of good bacterias, which benefit your overall health The only metabolite of kombucha that may be of concern to a pregnant woman is the alcohol content, which a serving on average has between 0.5% to 2/3% A non-alcoholic beer has 0.5% alcohol, so if you are having half a glass a day you are within your daily safety limits - but you have to make that personal conscious decision on what feels right for you As a breastfeeding woman that level of alcohol is considered to be a non-issue because the alcohol content is diluted It would take some work to get drunk off of the alcohol content in kombucha, and the trace amount in breastmilk is considered to be safe If you are buying your kombucha from stores those are quality controlled and you are not being exposed to bad bacteria If you are making your own kombucha it is important to check home-brew resources to understand how a SCOBY would mildew and how to prevent it from happening (21:14) Questions & Answers Jessica - What are alternative options to formula when one is dealing with low supply? Please be sure to listen to previous podcasts where this topic is also discussed: episode 4, episode 33 Stacy notes that breastfeeding is very personalized and is driven by society and what medical experts can help you with, please be sure to surround yourself with supportive people both in a professional capacity and in your friends and family circles Stacy's common suggestions regarding supply would be stress management, adequate sleep, increased fat consumption, omega 3 rich foods, teas like Mother's Milk, grain-free lactation cookies It sounds like Jessica did all of these things, but it sounds like there was a genuine medical situation in this case that may be impacting her ability to increase supply The only medical condition that Stacy is familiar with that impacts milk supply is thyroid conditions, and Stacy suggests looking to Practical Paleo on how to address thyroid issues Stacy notes that if your baby has a latch issue that is impacting supply, there are tube systems to help you through the latch challenges The Westin A. Price Foundation has some recipes for homemade formula From Stacy's experience, the flavor variations from the homemade recipes are of benefit to how your child develops their eating habits later in life Circling back on the thyroid medical concern, Sarah is not sure if a thyroid condition is the only thing that can impact milk supply, but it is a candidate If there is a low thyroid function the autoimmune protocol would be a good route to explore Sarah recommends pastured egg yolk, grass-fed ghee and lots of seafood (check this post from Sarah on mercury content) for those who are dealing with an autoimmune condition while pregnant Stacy reminds Jessica to be proud of all the effort you put in and to remember that you did the very best you could - your child can still lead a healthy life Like all things in life, be positive and find what things you can impact, like first foods, etc. (38:18) Kelly - how do hormones from breastfeeding impact adult acne? How do the hormones impact menstruation? Sugar may be causing the breakouts, which is a common issue that people face, it may help to eliminate sugar first Getting your baby enough milk is your biggest priority, so focus your elimination diet priorities so that you still get the abundance of micro and macronutrients that you need to keep your supply strong and rich in nutrients Sarah notes that there could be a hormone connection - so many things change during and after pregnancy from the hormone fluctuations, and the hormones could be impacting food sensitivities, the functioning of the immune system and the strength of the gut barrier Unfortunately there is not a good way to test what is causing your experiences other than dabbling in elimination dieting Stacy has shared on her hormonal changes from weaning and you can find that post here (49:40) Heather - what liquids should I provide my baby as I cut back on her breastmilk intake? Stacy and Matt replaced breastmilk with water, the kids never received anything other than breastmilk in a bottle Eleven months may be a little too young to wean because babies still rely on breastmilk at that age to meet all their nutrient needs You could try bone broth in a sippy cup for more nutrients In the beginning, Stacy and Matt put a very tiny bit of apple juice in the water to relate the flavor to the sweetness of breastmilk while transitioning them to drink water If you are taking breastmilk out of the equation, make sure you are adding things into the baby's diet that replace the nutrient density of breastmilk (think super foods like organ meats) Sarah notes that there is a societal expectation to wean at a year and Sarah encourages Heather to explore extended breastfeeding benefits and baby-led weaning to see if it could work for her family Stacy suggested eliminating the at-work pump sessions first, to cut down nursing time to night only so that her baby will still receive the breastmilk benefits When Sarah weaned her older daughter they dealt with a challenging case of thrush, whereas her younger daughter gave up breastfeeding without any difficulties - both of Sarah's kids were over two years old when they weaned (1:03:24) The reviews in iTunes are awesome and Stacy and Sarah love them!
