Ep. 214: Test, Don't GuessIn this episode, Stacy and Sarah discuss testing your suspected ailments instead of just guessing based on symptoms. So apparently the theory of menstrual synchrony is hard to actually verify and might not actually exist. The topic of today's show is "Don't Self-Diagnose!" It could be dangerous! Example: Adrenal FatigueThe symptoms for over active adrenals and under active adrenals can be very similar. You need to test cortisol to see what kind of problem you have. Don't guess based on symptoms There are many Adaptogens, but you need a testing practitioner to figure out exactly what you need. And the cortisol test is a neat spit test as well. Another example: SIBO (Small Intestinal Bacterial Overgrowth)There is a breath test and a stool test to determine if you have SIBO Current thinking says that you can't treat it just with diet, you need to kill the overgrowth with antimicrobials. It seems that many people diagnosed with candida (a yeast) overgrowth based on symptoms actually have a bacterial overgrowth. Another example: Hormone ImbalanceThe symptoms of estrogen excess look very similar to testosteron excess. You need to actually figure out what your levels are! Food sensitivities may be the exceptionPeople want the answer on a test to prove they should eliminate a food entirely. You can still be sensitive even if a test doesn't flag something. There are also foods with no tests at all. If you still react to a food when a test says you don't have an issue, why would you continue to eat it? Question from Hanna (18:33): "Do you strongly recommend blood tests for figuring out what you should and should not eat? I follow a paleo diet but I don't know if I should adhere to anything else like AIP."If you have a leaky gut, then you likely have multiple food sensitivities that will be hard to sort out by food journaling In this case, a sensitivity panel might be very useful as guidance. But if you're self-diagnosing and diet combining without real evidence for why you should, you're probably going to have a difficult time. Maybe you're not feeling your best because of what you're not eating. Try adding more nutrient density to your diet. Once you remove all these foods, you no longer have a health promoting diet. Question from Dani (26:29): "Asked for my thyroid to be tested by my PCP bc I had concerns I was dealing with a potential Hashi's situation (my mom has it, plus hand and finger numbness, hairloss, anxiety) but my Free T4 and TSH#s came back in the middle of normal range. B12 in the middle of normal but vitamin D is deficient (no surprise). For my anxiety my PCP Rx'd me an SSRI. Considering I've had hand tingling in the night, hair loss, and anxiety could this not be just vitamin D but really a thyroid issue despite my test results? Trying to figure out what next steps should be and what I should push back on my PCP for. You ladies are my faves...never miss a podcast!"It would be best to get a full thyroid panel. This site lists what should be on the panel Sarah covers her thyroid saga in this post. You can also have conversion disorders that are different from thyroid issues. Stacy and Matt went to Morimoto (home of Iron Chef Masaharu Morimoto), which she had done previously on her book tour with Sarah! Hope asks (13:38): "There are times when I choose to or have to eat out (food allergies here) and despite being very careful with what I order and informing the server/kitchen staff, something goes wrong in my digestive system (and I know it within minutes)...what can I take/do to lessen the negative effects and start getting my system calmed down and back on track? I know there isn't a magic bullet so to speak, just hoping for some advice on some things to do/try."Stacy says that her reactions have lessened over time, so there is hope! Chris Kresser talks about the bell curve of healing: at first you don't recognize your reactions, then they become so severe as the stand out more, then after healing they aren't as bad. There is a difference between food intolerances and allergies in terms of recovery:Intolerances have a shorter immune "memory". You may experience a lesser reaction after only 6 months to a year. Memory for allergies is much much longer. If your reactions are due to gut health, then healing the gut might make them go away! Some reactions just will never go away. And that's when you learn to live with it. There is a new product call Glutenza that helps with gluten exposure developed for celiacs who are accidentally exposed to gluten.Has digestive enzymes, prebiotics and probiotics that are intended to digest gluten and break it up before your immune system reacts to it. Digests 99% of gluten in an hour so that reactions are reduced. Intended for celiacs to make cross-contamination less of a problem, not so they can eat bread! Sarah uses it and it has not had reactions. Stacy's mom is anaphylactic to gluten and has learned to scout her food very well. Maybe if you're reacting to foods, your digestive system isn't optimized. See this post on Stacy's digestive system healing There are ways to increase your stomach acid. Supplements (HCl and Ox Bile), adding lemon juice or apple cider vinegar to your water, chewing well and not drinking during eating. How do you talk to a wait staff/kitchen staff?Do research before hand! Find a place that is allergen friendly. We us Yelp and FindMeGlutenFree. You can't ask questions of someone who won't know the answer! Some places will be more friendly than you expect. Stacy found that the Italian restaurant Maggiano's sent out the chef who custom made a meal for her around her intolerances. Know what to ask and make sure you're talking to an educated person. Don't be embarrassed! You're paying for a meal and they want you to be happy so you'll come back. They want to help you! Come up with a joke or a line to break the mood. Stacy always says: "We're high maintenance but we tip well!" Be clear and thorough with what you're looking for. Describe what you're looking for. Let them get excited about making you something special! Most waiters aren't going to be frustrated. If you get bad vibes and can't trust the restaurant, walk away! Recovery! What if you are exposed and need to heal?Sleep and rest and hydration. Like you have the flu! Nutrient density: organ meat, vegetables, broth, seafood, and vegetables. She means a lot of vegetables. She also has cystic acne, psoriasis and other skin issues that improved when she changed her lifestyle Stacy had bad acne and Finian has bad eczema before paleo. We recommend watching what you eat and what you use and what you put on for reactions. You can definitely see reactions when you track them! Sarah can't eat eggs without break outs while Stacy can't eat nightshades or corn Question from Katie: "Most of my AI symptoms have shown great improvement from my dietary changes. My energy is great, my joint pain has subsided, I've lost over 30 lbs, and my inflammation has gone down so I can wear my wedding ring for the first time in a few years. But...My psoriasis has gotten significantly worse! It is now on my face and is on my eyelids and under my eyes. It is also all over the back of my neck and my scalp is terrible. It is so painful and itchy. My integrative dr suggested calendula ointment which helps temporarily. Since you had psoriasis I thought you might have some other suggestions. I really do believe I am on the right path with AIP but desperate to find relief for this! Thank you!"Sarah says that it took 10 months to see psoriasis improve. And that certain aspects really helped: organ meats, vegetables, stress reduction, sleep improvement and probiotics. Retinoic acid (Vitamin A), and Vitamin D are important for skin health. These are fat soluble vitamins obtained from meat and especially organ meats Certain amino acids like arginine and glutamine are great for repairing skin. Vitamin C helps build collagen and fixes skin damage Zinc helps with the immune system Bromelaine helps reduce bruising and swelling and healing skin. Gut health is super important for skin issues as well. Especially soil based probiotics (Go play in the dirt!). Skin is a low priority organ for your body. If you're not responding to AIP, go see a functional medicine specialist! What you put on your skin is also very important! Stacy began to take notice of what she put on her skin after meeting Trina Felber from Primal Life Organics. Gluten is in almost every skin product! Sarah didn't quite know what she wanted to do professionally when she started paleo because when she began she was too sick to science. Now she's a health educator, thanks to paleo! She found a new part of herself that was strong and driven and able to be a positive and changed person. Stacy noticed how much she's changed from her family photos Sarah says she has found her voice and feels confident for the first time. Funnily enough, she's now learned how to do her hair and makeup! But both Sarah and Stacy emphasize that it's important to love yourself always! Love both your past and your present. Sarah got to talk at the Paleo FX Entrepreneur conference. She talked about how to have an authentic business. Stacy finds how looking at Facebook memories reminds her of how far she's come. Her slumps now aren't as low as her good days back then Sarah partially made her choices to model healthy choices for her daughters, who now see her modeling good choices. Remember: Every step is a good step on a long journey! Sarah won't be bored; she's got a dozen books she wants to write! Stacy wants to open a bed and breakfast in the future! The doctor misdiagnosed as Norovirus On Friday, when it didn't get any better and he was in a lot of pain, Matt took him to the emergency room. He was quickly diagnosed with appendicitis, and was sent to surgery. His appendix had burst by the time they got to surgery. A ruptured appendix is a medical emergency and immediately life threatening. Cole recovered quickly, "a medical miracle"! While Stacy credits his recovery to our lifestyle, we definitely followed the protocols of modern medicine to prevent sepsis and other infections We've been walking and eating healing foods for him. What is the appendix and appendicitis? (11:17)The appendix is a little "cul-de-sac" off the large intestine Possibly a reservoir of beneficial bacteria for the gut as a reserve if the gut microbiome is wiped out. -itis means inflammation, so appendicitis is when the appendix becomes severely inflamed, filled with pus and will possibly burst. Generally, if it becomes inflamed, it will eventually burst. It can be various causes, including genetic or infectious causes. It's not very well understood. Not considered to be the cause of any specific lifestyle or diet factor. We consider paleo to be a complimentary healing factor in this case (17:47)High dose antibiotics will affect the gut bacteria, so probiotic and gut healing foods like broth will help with recovery. Cole hasn't been eating well, and doesn't like the big pills he needs. So we've been finding foods he will eat first, including puffed rice cereal We have been making him Kombucha Gummies Stacy and Sarah both eat broth rice as well for sick kids It is a journey. You might not always be perfect, but it's important to try. Stacy believed that if we were paleo, the boys wouldn't ever need to have things like appendicitis, but paleo is not a panacea. The consequences of not having an appendix are very minor and having it out does not have a huge effect. Thank you for listening! So many people are missing sleep as the crucial component of their healing, a theme we've hear many many times. Sarah wrote the book about sleep. Literally. More and more studies are indicating that sleep is one of the most important health factor, and certainly the most neglected. One night of poor sleep causes more insulin resistance than 6 months of bad diet. Crazy! Good sleep helps with everything! From mood to stress to health choices! Bad sleep habits is as detrimental to your health as being obese, being sedentary or having a bad diet. And the trend of disease matches the decline in sleep amounts over time as well. Everyone seems to be using stimulants to cover up for sleep deprivation, and then keep us from sleeping well at night. It's affecting our health! Check out Matt's post, Propped Up By Stimulants Of course, don't feel badly if you're not getting your sleep right now. Things happen! But keep in mind what will be really beneficial for your health. With a new baby, sleep when the baby sleeps is great advice. Your baby won't benefit by you being a superwoman if it means you're too tired and sacrificing your health Kelly says, "Hey ladies love the podcast! I keep seeing people wearing amber glasses before bed. Does that really help with sleep?" (29:13)Mark Sisson came on the show and talked about amber glasses for night time in Episode 27! Get yours here! Amber glasses do work! They help you go to sleep quicker with better quality. They work by tricking your body and brain into thinking that you're in twilight. Meanwhile, our lightbulbs don't convince us that it's day time or its night time. And our computer and TV screens make us think it's day. Get F Lux for your computer Wear them two to three hours before bed to help with dim light melatonin production that is triggered by sunset. Shannon says, "I feel like you have talked about a favorite or recommended mattress brand in the past but I can't find anything on your site. Yes, me and my brothers all raced through dinners. But it was because there was only ever enough to give seconds to two or three of us! And we liked eating! And there were 4 of us! Ep. 204, Stacy's Physical and Mental RecoveryIn this episode, Stacy opens up about her struggles since her back injury last year from lifting a bag of concrete in a garden and how she is doing now. Real talk about depression, medical intervention and managing a huge life change. News and Views (0:40)Stacy and Sarah discuss how their Seinfeld hello is not their natural way to talk to each other. Stacy is nervous about this show, but Sarah is confident it will be amazing Stacy's injuryLast summer she hurt her back and has had a lot of false starts and dead ends in her healing. In the end, she was diagnosed 11 months after the actual injury. Surprisingly, it wasn't Strongman that injured her. In fact, it was wrestling against United States Strongest Woman Viv in a different sport (MAS Wrestling) and lifting a bag of concrete in a garden that did it. Backs like to be moved up and down, not side to side and twisting. And the MAS wrestling is a LOT of twisting. Stacy was recovering from that original injury and lifting again, but her pride in a community garden event got the better of her and she decided to try to lift a waterlogged bag of concrete to prove a boy wrong. That was the biggest injury. Her diagnosis in herniated discs in her 4th and 5th lumbar discs. The fifth lumbar disc is impinging a nerve on the right side. This is a very common "gardening" injury. Because of her "redhead" mutation, Stacy has a high body pain tolerance and a resistance to many pain killers (This is true!). So part of what made her treatment delayed was downplaying how much she was in pain. Her injury ended up flaring severely after she returned from Europe after a 12 hour flight, travel stress and unusual diet. This led to a freak out in the doctor's office, which led to her actually getting the MRI and the diagnosis. What happens to your life when you have a bad injury that changes your life?Stacy had really built her life around strength training. She gave talks, made friends, got a lot of benefits from it. She went from entering a competition she had a great shot at winning to not lifting for a year. Her quality of life was severely affected. At first she tried to cope by focusing on getting to relax, but after prolonged time away from the gym, it became quite depressing to see her friends getting better and her being unable to participate. Everyone who knew Stacy understood how much Strongman had become a part of her identity. And then the fact she couldn't do it anymore was crushing. Luckily, her injury is treatable. It can be treated with physical therapy and other medical intervention or, as a last resort, surgery. But there are some complementary interventions as well.Back injuries are difficult to heal because they don't take up nutrients the same way other body parts do. There is no blood flow. She'll be doing physical therapy to correct muscle imbalance and relieve pressure on disc. Sarah looked into nutrient deficiencies related to back issues and what supplements could possibly assist. Because disc get all nutrients from diffusion, you have to be very nutrient sufficient. Discs are made with very hard collagen on the outside and gooey center. Sarah recommends the Type 2 collagen from Vital Proteins because Type 2 is what makes up the disc. She also recommends Glucosamine and Chondroitin supplements. Also, Vitamin D deficiency is related to disc injury as well. Less well supported is Vitamin C and Zinc and Fish oil for pain management. Originally, she tried to find things to increase blood flow, but realized that wasn't possible. One of the side effects of this huge injury and life change was serious depression.Not only the loss of her sport, but she also was upset with herself, blaming herself for being lazy and making it up to be worse than it was. She was also very irritable and angry because of her chronic pain. He desire to binge eat returned as well. She started seeing a psychiatrist to deal with her issues. She went on an SSRI to help with her mood and binge eating. The diagnosis led to a lot of self loathing and feelings of failure. She cried for many days and didn't get it filled right away. She ended up deciding to take medication because she wanted her quality of life back, she deserved it and the people in her life deserved it, too. Returning from the doctor and confronting those feelings led her to do searches on paleo and depression. She found a worrying article (and frankly irresponsible article) by Chris Kresser about the "dark side" of antidepressants. She was doing all the things that should help restore her and they weren't working! If you've done everything and still not feeling well, then you deserve a great quality of life and shouldn't feel guilty about taking a pill! Going on the medication made her get her quality of life back! She now feels great! If you're going through something, don't feel like a failure if you have to go on medication. YOU ARE NOT A FAILURE! YOU JUST NEED A LITTLE HELP! The Paleo View always talks about how you should combine both the best of nutritional and lifestyle changes with the best of modern medicine. This definitely includes mental health as well. In paleo, there's often a bias against medicine and doctors because people come to it out of frustration. But medicine absolutely has its place, if you use it in balance. Sarah feels that now "old" Stacy's back lately. We hope that if you listen to this show, you use it as a catalyst to try to help yourself and the people in your life that seem to be a little not themselves. Cyndi has a fascinating history!Her mom had 7 brothers, all with haemophilia, which she suspects might due to lead chemicals on the family corn farm in Iowa But the Australian branch did not have health problems and Cyndi wanted to figure out how with an elimination diet So many health benefits right away! The best she'd felt in a decade. She realized that wheat was the number one issue with her and went on to research and discover why that is It's a part of Sarah and many people's health journey to understand what factors contribute to your health. The movie ends on a positive note of "just cook tasty food! It's easier than you think!" Everyone is influenced by Fat, Sick and Nearly Dead Rodney Ford was the only person she could find around Australia talking about gluten and wheat. Everyone was so fascinating that she included extended interviews with all the experts. All the experts were given the same questions, but gave very different answers. When she interviewed Stephanie Seneff, her mind was blown, particularly about the shikimate pathway.The shikimate pathway is a metabolic process that every lifeform except animals use to create certain amino acids. Because animals cannot make these amino acids because they cannot do the shikimate pathway, they are essential amino acids that must be obtained in food There were so many topics that there wasn't time to cover in the documentary. Maybe a What's With Wheat 2?! Sarah is a big fan of documentaries for fun, so it was special to see herself in one. Everyone had a great message and perspective, and one question that they were an expert in. Cyndi is constantly combating skeptics who doubt that non-Celiac gluten sensitivity exists (she often links them to Dr. Fasano). Changing Habits has a series of education programs to help people to learn because learning, particularly in diet and nutrition for doctors, will be the key to spreading the healthy message. Why can't we have our health message go viral when, for example, Ancel Keys was bad and incorrect health message that went viral as anti-saturated fat propaganda. Continued studies find different information when further research is done, but we don't quickly update our understanding. Only 8-11% of grants are funded so research is very limited. Particular for following up of hypotheses. And scientists are overworked as well as they try to keep finding research that will be funded with the limited funds available. 100 hour weeks hurt Sarah's health. Also, science is being misrepresented by sensationalist media. Remember: generally one study won't change the overall scientific consensus. Saturated fat does cause insulin resistance, but there's a evolutionary reason for it. Generally, you'll find for every weird quirk of the human body, there's a reason it got to be that way. Definitely triggered Stacy "trying to get away" with foods with that one apple or banana. Sarah concerned about going too low carb. See her take on potential adverse effects here. See Stefani Ruper on what signs indicate you need to eat more carbs. People often dive into a 21 Day Sugar Detox and then dive into sugar after. It also seems people "wing it" based on rumors about the programs rather than getting the straight information. Diane has carb recommendations for active people in her program, for example. These challenges have been an entry point for paleo for many people!We hear from many people who go on and off challenges and see paleo as a yo-yo diet, but that's not what's intended! Stacy says if you do more than three challenges in a year, it probably says that you have disordered eating patterns When first going paleo, it was months before either Stacy or Sarah even tried to make a paleo treat, which was a key to their success. Stacy and Sarah prefer the Whole Life ChallengeIt's a points based system that rewards you for lifestyle factors as well as food choices. You can be honest and take point reductions when you choose not to follow the plan strictly. Lifestyle factors can count for more than just a simple food choice. Factors include: stretching, water, meditation, etc. Stacy has posts here and here on WLC Online system in a group setting. You compete against your own goals. There's also a fitness test at the beginning and end that will allow you to see your progress. There's a little bit of a game aspect with the point system that gives you more immediate rewards. People are individual: Stacy eats some heavy cream, but no popcorn, for example, but Sarah is the exact opposite! The similarities between philosophies are greater than the differences. His anti-green bean and anti-potato stances and anti-salt are not generally accepted The Cordain stance on lean meats is due to the idea that most meat isn't grass fed or game meats, thus not what humans are designed to eat, but he's simplified to lean meats to try to be more understood. Early paleo focused on the whys of paleo, but Mark was one of the pioneers who talked about building a sustainable lifestyle. People who think paleo, think challenge. Then people enter paleo with a strict challenge mind set Whole 9 Life/Whole 30Their approach is a very strict approach inside of the challenge, but outside of the challenge are much looser. Throughout these past 200 weeks, nearly 4 years, a lot has change both in ourselves and in the world of paleo and in the greater world around us! Almost four years! Let's celebrate! And we still haven't talked about everything! Let the show continue! We've seen so much change in this movement, not just over our four years, but in the 15ish years since the Paleo Diet was first published. And when you think about the first iteration of this diet compared to what it looks like now, the progress is just amazing! Stacy has been paleo for just over 6 years, and Sarah is approaching 5 years on paleo Stacy did a paleo timeline post that she often looks at to remind herself of her evolution. Sarah has come so far in her paleo journey!The things that made Sarah's life hard pre-paleo transcended food: depression, Irritable Bowel Syndrome, tantruming child, a child with obstructive sleep apnea, joint pain. Now, she's able to sit in her daughters' dance recital without getting tired or pain. Paleo has given her sufficient sleep, and strength and healthy kids and is able to live in the moment. Before Paleo, Sarah was barely surviving motherhood. Her original goal was to clear up some skin issues, but it's given her so much more! Stacy's life has improved so much thanks to paleo, too!Stacy's goal was only to have enough energy to play with her kids. A doctor told her she'd be less likely to die at an early at an early age if she lost 20% of her 300+ pound body weight She's been to the pool everyday this week with the boys. Recently she saw an overweight woman in regular clothes trying to play with her kids on the sidelines and thought that it could have been her. Meanwhile, she's playing with the kids in the pool and doing laps. Paleo has been transformative! When she describes why she's paleo nowadays, she doesn't immediately jump to food and weight loss, but talks about health benefits. Like Cole being off his inhaler, and Stacy being much healthier now. Sarah is happy that she is health enough to not be bypassed for exercise activities.Sarah is happy to have flexed with Jen Sinkler and felt a part of the healthy group. No one is old! Being in your 30s is not old!Why is it that only old people swim laps at the pool? Stacy was declared by Cole, age 10, as "not that old" and that old age now starts at 60 for him. Sarah's daughter is 6 and believes that Sarah is 78 years old. Meanwhile, Stacy is getting treated as her own father's wife. (This is because she is the trophy wife, I swear) The Paleo movement has changed and grown so much!At Paleo Fx, people were quoting a population of 10-15 million people consider themselves paleo. Paleo started with The Paleo Diet by Loren Cordain in 2001. When Stacy first checked it out at the library in 2010, it still had the publisher mandated recommendations of Diet Coke and canola oil, which were rectified in the 2010 revised edition. Stacy started paleo even before the second big paleo book, The Paleo Diet Solution by Robb Wolf, was released. Stacy read a lot of Paleo Hacks (Often a wretched hive of scum and villainy. You must be careful there!) back in the day. She remembers being terrified by a post that was decrying that some "New World" foods like nightshades were being called Paleo. This scared the crap out of her! At the time, she declared that she could NEVER give up nightshades! She found comfort in Elana's Pantry and Spunky Coconut making recipes that could be adapted to paleo Paleo Parents came about during a time when all the bloggers were just fumbling in the dark because there were no resources Sarah came in before It Starts With Food, but when Whole30 was still a powerful force. Paleo back then was very much low carb, no potato, low fat. Over the years, Paleo has acquired a lot of new researchers and voices in the movement like Mat Lalonde, Alessio Fassano, and Stephen Guyenet (who is my boy). We're moving from broad brush strokes, to more fine tuning into what an ideal diet might be. Healthy diets now seem to be rich in healthy fats and are micronutrient sufficient. The starting point is still cutting out the bad stuff, first, but then there are lots of variation. People are coming to their own conclusions based off of numerous sourcesPotatoes were beginning to be accepted by many bloggers, but the big turning point was being accepted by Whole 30. There's no central leadership in paleo to make declarations, Whole 30 was, for a long time, the people that made these decisions for a lot of people. These days, sometimes it trickles through, like with green beans. Dairy products are definitely controversial for many people. There's a movement to include A2 Beta Casein milks like camel milk. Different thought leaders have different thoughts on paleo approaches, which become the focal points for big rethinking. The Paleo community isn't about what's right and wrong, paleo and not paleo, but what will work for you. Paleo is founded in our scientific understanding of what works for humans, as well as what genetic variation will make individual bodies work best, which empowers people to make decisions for themselves to create a healthy sustainable lifestyle Episode 200 is done, what will we do for our 4 year aniversary?Our Fourth Anniversary show will be on August 12th, 2016 Sarah and Stacy get schmoopy!
