Point-of-care ultrasonography in patients admitted with respiratory symptoms: a single-blind, randomized controlled trial. The title is a mouth full and when you read it it’s definitely a brain full.  This is an incredible study with some incredible results.  If you just read the abstract you see that the correct diagnosis is made 88% of the time within 4 hours with ultrasound used and only 63% without ultrasound.  BIG WIN! However, when you read the paper, you notice some very head-scratching things.  There may be trends towards more diagnostic testing, more treatment, and possibly increased mortality and morbidity when ultrasound is used.  So a lot of people have talked about this and wondered if it was real. Well, we wanted to dive even deeper and talk with the author of the paper.  Let’s dissect the paper and discuss what wasn’t in it.  What are we missing? You’ve got a little bugger with belly pain and they may have intussusception…..or they could just have gastro.  How to know?  Well, this isn’t the xray podcast.  Of course, you’re going to ultrasound them.  Or if you’re not, watch this and hopefully you will afterwards. Turns out ultrasound for intussusception isn’t that difficult.  There’s pretty good research on point of care providers doing it, and we’ve got Russ Horowitz from the University of Chicago on to show you how. OK, so here it is. Rob’s gonna show us exactly how to do this.  How to take those 800 normal Transesophageal Echocardiography (TEE) views and distill it down into a few key views.  We know you aren’t doing this yet, but we think it’s coming.  Just a matter of time.  Get ahead of the curve. And yes, you read the title right.  The EDECMO folks, Shiner, Bellezzo, and Weingart will be teaching an ECMO lab at Castlefest.  Sure, it’s not really an “ultrasound procedure”, but we’re obsessed with this cutting edge, futuristic technique of saving lives that couldn’t be saved otherwise.  So we convinced them to come out to Castlefest, mainly so we could learn it from them, but if you’re coming, you may as well learn as well.  We’re super pumped about this!  You can sign up on the Castlefest website now.  Oh yeah, Rob will be there again teaching TEE.  We’ll run some ultimate simulations with ECMO, TEE, and all kinds of things that make us as excited as a tick on a fat dog. At Castlefest 2014, Rob Arntfield blew our minds with his point of care Transesophageal Echocardiography (TEE) talk.  He showed us why and how we could and should be doing point of care TEE.  Then we all practiced on simulators.  It was a phenomenal experience, and he’s agreed to come back in 2015 and do it all again.  So if you haven’t signed up, do that NOW! However, we think this is too important to just teach to Castlefestians, so we want to #foamed it out to all of you.  The truth is, this isn’t the hardest thing in the world to do.  It sounds scarier than it really is.  If you can get your hands on a simulator to practice, you can totally learn this……probably more quickly than TTE.  That’s the experience of most people who do this.  The windows are really much easier. It’s here!  Castlefest 2014 is this week!  We hope you’re as excited as we are.  We’re going to be covering a lot of cardiac ultrasound….pretty advanced cardiac ultrasound.  So we wanted to put out a quick podcast on some of the more basic, foundational studies of bedside cardiac US. This is short and sweet.  Very minimal and not at all supposed to be an in depth look.  It’s just a few studies that we think are important background to know when we talk about using echo at the bedside.  It’s definitely part of the “Back to the Basics” series.  If you missed the FASH episode on HIV/TB last week definitely go back and watch it.
